Funeral service at St Johns Church, Worcester
on Monday 23rd December at 11am,
followed by interment in St. Johns Cemetery.
Please wear colour to represent Sam's wonderful personality, you may also wear black if you wish.
If you wish to donate in memory of Sam, all donations will go to the work of both The Midlands Air Ambulance and The British Heart Foundation, who both mean a lot to the family.
Any donations may be left at the church or sent via this site.
