Allan JohnCOOKThe family of the late Allan John Cook, who died on Sept 27, 2015, wish to offer their sincere thanks and appreciation to all family, friends, professionals and agencies who treated and cared for Allan in hospital and at home. They include all staff of Ward D3 East, Royal Gwent Hospital, MAU (Medical Assessment Unit), St David's Hospice Care at home, doctors and staff of Bryngwyn Surgery, Newport, District Nursing team from Bryngwyn Surgery and Chapelwood, Fred Taylor and Wasan the Pharmacists at Marshfield Pharmacy, Karen Hazel and Sarah Cross, specialist nurses Cardiology, Dr Jackie Austen and Sister Helen O'Leary, Nevill Hall Hospital, Physiotherapists, Occupational Therapists and social worker at RGH, Father Michael Evans, Father Richard, Father Michal Doyle of St Mary's RC Church Newport and Monsignor Finbarr O'Leary from Ireland, Father Paul Millar, Sr Marie de Mountfort St John Lloyd Parish Cardiff, Tracy and Gordon Williams from the Re-ablement Care Team, Patrick Roberts, South Wales Stairlifts, Bluebird Care Agency, Mike the carpenter, Tovey Brothers Funeral Directors, Mr Clive Lemon, vocalist and friends and neighbours who visited and supported Allan and our family during his illness, a big thank you. Also to everone who sent cards and letters of condolence and donations in lieu of flowers for St David's Hospice Care. A Mass will be offered for your intentions.
